Waiver

SAVE TIME
If this is your first time visiting Ball Factory, or it has been more than One Year since your last visit, please save time by completing a waiver now before you visit. Once you complete the form, it will be automatically sent to our front desk and waiting for you to arrive.

All children ( babies, toddlers, teens) AND Adults ( 18+, parents, granparents, aunts, uncles, etc.) must have a signed waiver on file. For those visitors under 18, their parent or legal guardian must sign the waiver for the minor and themselves. 
 
A common misunderstanding is that adults do not need waivers unless they plan to play. 
In reality, every guest entering Ball Factory must have a completed waiver on file, whether they are playing or not. 
That means adults, even if they are only coming in to watch, chat, or cheer from the sidelines, the waiver still applies.
